Job Summary and Responsibilities

Job Summary

Make a meaningful impact in your community-join Acentra Health as a Clinical Evaluator (PASRR) in Seattle, Washington. In this role, you will conduct PASRR Level II Preadmission Screening and Resident Review assessments to help determine appropriate placements and next levels of care for individuals with complex needs. As a Clinical Evaluator, you will conduct behavioral health assessments for the geriatric population as well as individuals with Intellectual Disabilities (ID), Serious Mental Illness (SMI), or Related Conditions (RC) across nursing homes, hospitals, and community-based settings. These evaluations play a vital role in identifying the appropriate next level of care and supporting improved long-term health outcomes.

This full-time hybrid role offers both flexibility and purpose combining remote work from your home office with local travel. Acentra Health provides reimbursement for travel-related expenses associated with this role, including mileage paid at the federal reimbursement rate and covered parking. We provide all essential technology and equipment needed for your home office setup, including a company-issued laptop for use both at home and in the field.

Responsibilities

• Responsible for conducting PASRR Level II behavioral health evaluations, assess individuals' physical, behavioral, and psychological functioning, and develop informed level-of-care recommendations to determine the most appropriate next level of care for individuals who may require long-term care placement.
• Assess and analyze patients' psychological, emotional, behavioral, and physical functioning to inform clinical decisions.
• Review medical records, and interact with healthcare professionals, patients, their families, and other support systems to conduct accurate screenings, evaluations and determinations.
• Provide well-informed level of care recommendations based on clinical evaluations, ensuring adherence to established workflows, including re-evaluation of records in response to psychiatric input.
• Leverage technology by utilizing the state's electronic system and our proprietary platforms to efficiently manage and track all assessment activities.
• Foster effective collaboration with a diverse range of healthcare professionals, patients, families, and support networks while monitoring personal metrics to ensure compliance with turnaround time requirements.
• Knowledge of standards of practice related to Level of Care determinations, as well as mental health, geriatrics, and other conditions commonly requiring long-term care placement.
• Ensure compliance with established workflows, including re-evaluations of records based on psychiatric input.
• Understand, and comply with all corporate policies, including those related to HIPAA and its Privacy and Security Rules.

Qualifications

Required Qualifications and Experience

• Must possess an active, unrestricted license in the State of Washington in a healthcare or related field, such as Licensed Psychologist (LP), Licensed Professional Clinical Counselor (LPCC), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), Licensed Mental Health Professional (LMHP), Registered Nurse (RN) with Master's degree in Psychiatric Nursing or relevant field).
• Graduation from an accredited college with a relevant degree program and a minimum of 3 years of experience in assessing individuals with mental health or psychiatric disorders, including complex differential diagnoses related to substance use, medical conditions, cognitive impairments, and dementia.
• Proven ability to assess and analyze psychological, emotional, behavioral, and physical functioning to support accurate and thorough clinical evaluations.
• Experience in medical record abstraction and clinical data review.
• Strong communication skills, with the ability to interact effectively and with empathy with individuals and groups through verbal, written, and electronic communication.

Preferred Qualifications

• Basic knowledge of Preadmission Screening and Resident Review (PASRR) requirements, including compliance with state and federal regulations.
• Familiarity with mental health, intellectual and developmental disabilities (IDD), geriatrics, and other conditions commonly requiring long-term care services and supports.
• Knowledge of DSM psychiatric disorders and Level of Care standards, with experience diagnosing conditions using DSM criteria (Washington State-specific)..
• Previous experience working with the geriatric population.
• Detail-oriented with excellent organizational skills and the ability to prioritize and complete tasks efficiently within established timelines
• Proficient in computer-based software and tools, including Microsoft Office Suite (Word, Excel, Outlook, Teams).

Compensation

The compensation for this role is $89,440.00 - $93,600.00 annual base salary

Based on our compensation program, an applicant's position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
