The RN Certified Diabetic Educator will provide diabetes education conducive to behavior change and improved quality of life across the lifespan. The RN Certified Diabetic Educator supports and advocates for people affected by diabetes to optimize quality care. The RN Certified Diabetic Educator promotes self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes. This position reflects the mission, vision and values of the practice both internally and throughout the community. Education/Certification/Licensure NYS Registered Nurse (Bachelor’s Degree preferred) Certified Diabetes Care and Education Specialist or Board Certified Advanced Diabetes Management credentials. If not already a Certified Diabetic Educator, will sit for the certification exam within 24 months of hire.
