Overview In the heart of The Dalles, Oregon, Adventist Health Columbia Gorge has served the Columbia River Gorge community since 1901. Job Summary Provides Diabetes education for inpatient and outpatient diabetes patients, pregnancy-related diabetes patients, and/or nutrition clients in individual and group settings. Assists with the planning, developing, implementing and evaluating of diabetes education programs to ensure compliance with established goals and the realization of quality, economical health care service. Demonstrates the ability to meet the educational needs of the diabetes patient. Job Requirements Education and Work Experience: Bachelor’s Degree or equivalent: Required Master\'s Degree: Preferred Licenses/Certifications: Registered Dietitian Nutritionist or Registered Nurse (RN) licensure in the state of practice: Required Certified Diabetes Educator (CDE): Preferred Registered Dietitian (RD) or Registered Nurse (RN): Required Facility Specific License/Certifications: Basic Life Support (BLS OR HS-BLS OR RQI BLS) certification: Required Essential Functions Plans, develops and implements individual and group diabetes inpatient and outpatient education for adults, pediatrics, and their families and caregivers. Maintains in-depth knowledge and understanding of the nature of diabetes and related issues and patient care concerns. May be responsible for diabetes program coordination as determined by local market. Collaborates with the client, significant others, health care providers and team members. Collects and analyzes specific client data and outcomes. Assists in the development and implementation of educational and dietary counseling interventions based on client-specific needs. Maintains and trains with clinical expertise in care related to diabetes management and other chronic diseases. Leads self-management training and education to type 1, type 2, and gestational diabetic patients. Participates in the assessment and development of diabetes education materials and programs. Participates in program quality and effectiveness evaluation activities. Participates in program marketing activities. Performs other job-related duties as assigned.
